I've seen a few wheels up landings nearly all similar situations, pilot and aircrew distracted by a variety of things.
 Bird Strike - Zero
Flew into Shark Bay Airport, in Northern WA. There is one bloke there who is air traffic control (has a handheld radio), mechanic, baggage handler, dispatcher, re-fueller, and drives the passenger bus into "town"

The pilot had to buzz along the runway before landing, to scare the wildlife into the bush and then do a quick about turn and land before they came back. On departure "mr do it all" let of scarers just before take off.

I am reminded of the Billy Connolly tale of his fliying aboard a plane in Oz owned by a company called Emu Airlines...... the tag line of which was ......

......'and then I remembered - Emus canna fly'........
